Below are the 2011 Krewe of Gemini Order Forms (Adult & Kids).  I am sending electronically in order for you to have the ability to print your hard copies as you need.  If you have problems I will have some hard copies at the Monday meeting

2011 Bead Order Forms:

The form is somewhat self-explanatory but let me highlight some changes:

• The only required items are Krewe Mug, White Gloves, and Float Polystones.
• We have added a generic package to the order form this year at the request of riders.  It is just an option.  Example: if you want to order the package and be done that is all you need to do OR you can order the package and supplement with additional items. You can also skip the package and select what you want to order from the listed items.
• Payments: If you are paying by check and making two (2) payments, I require both checks to be included with the original order.  First payment to be processed when collected with the balance payment held and deposited on October 15.  This is the same process for credit card payments. 

You need to submit your order form to your float lts. If you are sponsoring a child rider turn their order into your float lts. (Same for teen) That way you can pick up the children orders when you pick up your orders.
